CDS – Consumer Directed Services
Consumer Directed Services (CDS) allows individuals who qualify for Medicaid to choose and manage their own caregiver—often a trusted family member or friend. The consumer stays in control of their care while our agency handles payroll, training support, compliance, and Medicaid billing. CDS helps participants remain independent in their homes while receiving assistance with:
• Personal care (bathing, dressing, grooming)
• Meal preparation
• Light housekeeping
• Mobility and transfers
• Errands and daily living support
This program gives consumers flexibility, choice, and control over who provides their care.

IHS – In-Home Services
In-Home Services (IHS) provides professional caregiver support for individuals who need assistance with daily activities but do not wish to manage their own attendant. IHS services may include:
• Personal care assistance
• Medication reminders
• Meal preparation
• Light housekeeping
• Companionship
• Safety monitoring
IHS is ideal for individuals who want hands-on support without the responsibility of hiring or managing caregivers themselves, while remaining safely at home.

ADC – Adult Day Care
Adult Day Care (ADC) offers a safe, structured, and engaging daytime environment for adults who need supervision, socialization, and support while living at home. Services are provided during the day, allowing caregivers and family members peace of mind. Our Adult Day Care services include:
• Supervision by trained staff
• Social and recreational activities
• Meals and snacks
• Health monitoring and nursing support
• Transportation to and from the center (when applicable)
ADC promotes independence, social connection, and overall well-being, while providing families with reliable daytime care.

PDN – Private Duty Nursing
Private Duty Nursing (PDN) provides skilled nursing care in the home for individuals with complex medical needs who require ongoing clinical support. Services are delivered by licensed nurses based on a physician’s plan of care. PDN services may include:
• Skilled nursing assessments
• Medication administration
• Tracheostomy and ventilator care
• Feeding tube care
• Chronic condition management
• Post-hospital or long-term medical support
