This updated guidance:

We’re used to thinking about PCSK9 inhibitors for patients who have already had a heart attack or stroke. But what about those who are clearly high risk and haven’t had that first event yet? 🫀

In a new Science Matters article, MOACC research fellow Michael Katsnelson, MD, PhD, FACC summarizes the VESALIUS-CV trial, which studied evolocumab in patients with atherosclerosis or high-risk diabetes but no prior MI or stroke.

Here are a few takeaways from his summary:
▪️ More than 12,000 patients were enrolled, almost all on background lipid therapy
▪️ Evolocumab cut LDL from around 115 to about 45 mg/dL
▪️ The drug reduced major adverse cardiovascular events (heart attack, coronary death, ischemic stroke, and ischemia-driven revascularization) compared with placebo
▪️ Longer follow-up (about 4.6 years) helped show the sustained benefit of aggressive LDL lowering

Dr. Katsnelson also raises an important idea: could future decisions about PCSK9 use be guided by overall plaque burden measured by imaging, not just LDL numbers alone?
