What is Transitional Care? Our Transitional Care program supports patients who have been discharged from the hospital after illness or surgery. Those former patients no longer require hospital care but are not yet ready to return home as they need the following:
• Personalized plan of care
• Regular meetings with your care team to help you and your family reach your goals
• Hospital-level nurse staffing to provide care and keep patients safe
• On-site physician, therapy, radiology, laboratory and pharmacy teams to address your medical needs

You may have heard the words “aphasia” and “dysphagia” when supporting a loved one’s health journey and wondered what they mean and how they differ.

June is National Aphasia Awareness Month—a fitting time to highlight this communication disorder and contrast it with dysphagia, another disorder patients may face after injury or illness.

Aphasia affects a person’s ability to speak, read or write—although not necessarily their thinking. After a stroke or brain injury that damages the language centers in the brain, a patient may have trouble finding the right words, understanding what others say or comprehending text.

Dysphagia is the difficulty in swallowing, and can occur as a symptom of many different conditions, including brain and muscle disorders.

When your loved one is critically ill or injured, you want them to receive compassionate care. Our hospital nursing teams care for patients who require an extended stay. They specialize in caring for patients following a stroke, spinal cord, brain and orthopedic injuries, neuromuscular illness or other medical conditions. With a patient-first spirit, our nurses are committed to quality, safety and positive clinical outcomes.

Do you know the warning signs of stroke? Use the acronym BE FAST to recognize symptoms in yourself or a loved one, and call 911. Quick medical attention can lead to a better outcome.

Balance – Watch for sudden loss of balance.
Eyes – Is there a sudden loss of vision in one or both eyes? Or double vision?
Face – Ask the person to smile. Does one side of the face droop?
Arms – Ask the person to raise both arms. Does one arm drift downward?
Speech – Ask the person to repeat a simple phrase. Is their speech slurred or strange?
Time – If you observe any of these signs, call 9-1-1 immediately.
