Dear Patients,

It’s with mixed feelings that I announce my retirement from the practice of medicine. As I approach the end of my career, I wanted to take a moment to express my gratitude for the many years I have spent with Stayton Family Practice. I have decided to retire effective June 6, 2023 and will transition to an administrative role to spend more time with my family and pursue my personal interests.
It has been a privilege to work for Stayton Family Practice for these past 15 years, and I appreciate the opportunities and support that I have received during my tenure here. I am grateful for the relationships with all my patients throughout the years.
As I move on to the next chapter of my life, I will cherish the memories and experiences gained throughout my career. I am proud to have been associated with Stayton Family Practice and will continue to follow its progress with great interest.
We will still have providers available during this transition. We are in the process of bringing on a new provider to take my place as well as take over the business, more details to come. Thank you for the opportunities and support you have provided me. I will miss working with all of you and I wish the company continued success in the future.

Dear Lebanon Patients,

I would like to thank you for the trust you have given me over the years as your physician. Your health and wellness is our top priority and it has been an honor to serve you and your family. I am writing to you today to inform you that due to the lack of staff and impact of Covid, I will be closing my Lebanon location 41 E Ash St. Lebanon, Oregon 97355 on February 28, 2023 after this date we can continue to provide care at our Stayton location.

All medical records are confidential and protected by HIPAA, federal privacy and security regulations. With your medical records release authorization form completed, a copy of your/family record (s) can be released to you or transferred to a physician. Complete, sign and return the enclosed medical record release form as soon as possible. Be sure to indicate how you would like to obtain the medical records and whether you would prefer them in electronic or paper form.

You have a few options:
You may pick up your/family medical record (s) at this practice
You may have them mailed to you
You may have them sent to a physician you designate.

*Please note there will be a $25.00 fee for physical medical record copies that our mailed or picked up*

I regret that I will not be able to continue to serve you through our Lebanon location. My years here have been filled with many rewarding experiences and memorable patients. Please know that I have greatly valued our relationship and wish you and your family the best of health.

Our Stayton office at 1881 W Washington St. Stayton, OR 97383 will remain open where we could better serve you through this office and virtual appointments are available. If you have any questions you may contact our office at 503-400-6140.

It brings us great sadness to announce that our Jefferson location will be permanently closed as of April 28th, 2022. Our Jefferson staff will be transferred to our Stayton location, and will continue to provide you with the top quality care you have come to know in Jefferson. On behalf of all of us here at Stayton Family Practice, we thank you for 13 wonderful years of operation and look forward to continuing you and your family's care at our Stayton and Lebanon branches!

It brings us great sadness to announce that as of April 28th our provider Troy Nies, PA will no longer be with our practice. If Troy is your current provider you may establish care with any of our other providers. We wish Troy the best of luck on his next endeavor! For those of you in need of an immediate appointment, our Lebanon and Jefferson clinics are still open for walk-ins and other appointments.
