Our doctors take a compassionate approach to the management of both short and long-term pain and the use of narcotic medications is an essential tool in the care of a patient in pain. We wish to alleviate suffering in its many forms. This goal, however, must be tempered with the credo Primum non nocere or “first do no harm.” The

vast prevalence of addiction in our communities has necessitated several policies to which our patients must be made aware. Because of the risk of addiction and the strict oversight of the State of Michigan, our practice’s policies are as follows:

1. On a first new patient visit, no narcotics or other controlled substances will be prescribed in the absence of a clear, acute, injury.

2. Patients requiring chronic pain medications must enter a written pain contract, and agree to use only one doctor and only one pharmacy.

3. Urine drug screens may be requested at any time.

4. Chronic pain management requires individual visits to address the pain and its treatment independently of other medical problems. Usually this requires a dedicated visit every 3 months, or more often.

5. Patients found in violation of pain contract will no longer be prescribed narcotics pain medication and may be discharged from the Practice.

COVID-19 Third Vaccine Dose Information

On Aug. 13, CDC/ACIP approved a third dose of mRNA vaccine (Moderna or Pfizer) for people who are immunocompromised. (The CDC/ACIP did not make any recommendations for additional doses of COVID vaccine for immunocompromised patients who received the Johnson & Johnson/Janssen vaccine.)

Eligible patients include those who:

Are receiving active treatment for solid tumor and hematologic malignancies or have a diagnosis of cancer in the past 12 months

Have received solid-organ transplant and are taking immunosuppressive therapy

Have received CAR-T-cell or hematopoietic stem cell transplant (within two years of transplantation or are taking immunosuppression therapy)

Have a primary immunodeficiency (e.g., DiGeorge syndrome, Wiskott-Aldrich syndrome)
Have advanced or untreated HIV infection (CD4 count < 200)

Are receiving active treatment with high-dose corticosteroids (i.e., ≥20mg prednisone or equivalent per day for more than 14 days), alkylating agents, antimetabolites, transplant-related immunosuppressive drugs, cancer chemotherapeutic agents classified as severely immunosuppressive, tumor-necrosis (TNF) blockers, and other biologic agents that are immunosuppressive or immunomodulatory.

Have chronic medical conditions such as asplenia and chronic renal disease, (dialysis patients and patients with proteinuria).

Who needs to quarantine?

People who have been in close contact with someone who has COVID-19—excluding people who have had COVID-19 within the past 3 months or who are fully vaccinated.

People who have tested positive for COVID-19 within the past 3 months and recovered do not have to quarantine or get tested again as long as they do not develop new symptoms.
People who develop symptoms again within 3 months of their first bout of COVID-19 may need to be tested again if there is no other cause identified for their symptoms.
People who have been in close contact with someone who has COVID-19 are not required to quarantine if they have been fully vaccinated against the disease and show no symptoms.
What counts as close contact?

You were within 6 feet of someone who has COVID-19 for a total of 15 minutes or more
You provided care at home to someone who is sick with COVID-19
You had direct physical contact with the person (hugged or kissed them)
You shared eating or drinking utensils
They sneezed, coughed, or somehow got respiratory droplets on you
Steps to take

Stay home and monitor your health

Stay home for 14 days after your last contact with a person who has COVID-19.
Watch for fever (100.4◦F), cough, shortness of breath, or other symptoms of COVID-19
If possible, stay away from others, especially people who are at higher risk for getting very sick from COVID-19

Chloroquine has been used for COVID-19 treatment but has not been approved by the FDA yet.
Hydroxychloroquine is the active drug so do not use any other Chloroquines, as you know an Arizona man died after ingesting Chloroquine Phosphate which is not the same drug and he died.
