For now and through the end of September, we still have someone at the office Monday through Friday, 9:00 AM—1:00 PM, for patients to pick up their records.
If you cannot come during those hours, please message us on here or call the office at 740-283-7100 between those hours and we will do our best to arrange an appointment for you to pick up your records at another time that works for you and one of our staff members.
We ask that you do this as soon as possible to ensure you are able to obtain your records before they are relocated to a secure storage facility per current HIPAA guidelines.

As many have heard, it is with great sadness we must share that Dr. Pat was unexpectedly called Home to his eternal resting place in Heaven. We have been trying to find the right words to express our grief, but there are no words to capture the emptiness we are all feeling. He will be sorely missed, and we ask everyone to keep his family, friends, and staff in your thoughts and prayers.

Dr. Pat loved every single one of his patients and would want us to ensure the continuity of care for them all. Please call or stop in to the office in the upcoming weeks between 9 AM— 1 PM, Monday through Friday, to organize the pick up of your medical records. Please call before you come to ensure there is someone there.

If you have any questions regarding the referral to another provider, any outstanding charges, or active prescriptions, orders, or prior authorizations, please call and ask to speak to Tori.
