It is with shattered hearts that we share the sudden and unexpected passing of our dear colleague and friend, Scott Forrest.
Scott was a gifted physical therapist who spent over 20 years at OSI’s Stillwater clinic, impacting countless lives with his care, candor, and unmatched sense of humor. He had a rare gift of making people feel seen - with a perfectly timed joke, a sarcastic jab, or simply by showing up and doing the work with heart. Scott was one of a kind - a fierce competitor, a loyal teammate, and a therapist beloved by his patients and coworkers alike. But above all, Scott was a devoted father. His son Logan was his world, and the love he had for him was undeniable in everything he did.
His passing leaves a tremendous hole in the OSI family and in the hearts of so many who knew him. We know this loss is felt deeply by many of you - patients, coworkers, friends, family and beyond.
🟆 A Celebration of Life for Scott will be held:
- Friday, July 18th at the Cloquet Country Club
- Visitation at 4 PM | Memorial Service at 6 PM | Reception to follow
- In true Scott fashion, guests are encouraged to wear a funny or inappropriate graphic tee, sports gear, or whatever you feel most like comfortable in. Keep it casual!
🟆 In honor of Scott, our OSI team is creating a memory book for his son, Logan - a collection of stories, photos, and reflections that celebrate the incredible impact Scott had on those around him.
- If you have a memory, a message, or a favorite photo of Scott from his time at OSI, we invite you to share it with us.
- Contributions can be dropped off at the front desk at OSI Stillwater or emailed to kfroemke@osipt.com or jwolowicz@osipt.com.
- We’ll be gathering submissions through the end of July. Thank you for helping us create something meaningful for Logan to cherish.
