āž”ļø The bigger picture most families don’t realize is there are four main levels of senior living:
Independent Living, Assisted Living, Memory Care, and Skilled Nursing.

Each one supports a different level of need. The goal is finding the right fit, not just choosing a place.

"Where do we go from here?"

After a hospital stay, many families are suddenly faced with decisions they never expected to make. This means the next steps can feel very overwhelming.

āž”ļø This is why the heart of social work matters.

Social workers are trained to look at the whole person, not just the discharge paperwork. That means understanding mobility needs, medication routines, emotional well being, and what daily life will realistically look like once someone leaves the hospital.

Structure and routine also play an important role during recovery. Predictable support helps seniors feel grounded and more in control of their day. Simple things like knowing when meals happen, gentle medication reminders, and familiar faces can make a meaningful difference.

Families feel it too. Caregiver stress often rises after a hospital stay. When there is clarity and the right level of support, families experience relief and peace of mind. Many tell us they finally sleep through the night again and feel supported by a trusted care team.

As a social worker with a background in hospice and home health, Joanna understands the complexity of discharge decisions, caregiver stress, and the benefits available through Medicare, Medicaid, and VA resources.
