Summary
As a Nurse Practitioner at Nurse Practitioner Health Services, LLC, you will be an integral part of our healthcare team, delivering high-quality medical care to residents in long-term and assisted living facilities. This role is vital in ensuring our patients receive the best possible care while promoting their overall health and comfort.
Responsibilities
Conduct comprehensive assessments of patients' health status.
Develop and implement individualized care plans based on patient needs.
Provide direct patient care, including diagnosis and treatment of acute and chronic conditions.
Collaborate with multidisciplinary teams to ensure coordinated patient care.
Educate patients and their families on health management and wellness strategies.
Monitor patient progress and adjust treatment plans as necessary.
Maintain accurate medical records in compliance with healthcare regulations.
Participate in quality improvement initiatives to enhance service delivery.
Qualifications
Valid Nurse Practitioner license in the state of practice.
Experience in hospice palliative medicine or acute care preferred.
Strong knowledge of behavioral health practices.
Excellent communication skills for effective patient interaction.
Ability to work collaboratively within a healthcare team.
Familiarity with health coaching techniques is a plus.
Proficiency in aseptic technique is beneficial but not required.

We are now offering two options for you.

BinaxNow Abbott Covid-19 individual test kits. TWO tests per box. Offered at our off site location (family farm in Columbia Station just outside of Strongsville). $40/per box OF TWO. No contact self pick up any time day/evening. Cash, Check or Venmo accepted. PM for details.

Flowflex Covid-19 individual test kit. $15/per box. ONE test per box. Available at our office 15400 Pearl Rd Strongsville, OH 44136. No contact pick up during business hours (M-F 9-4pm). Credit card or Venmo accepted. Call our office to order/pay 440-879-1108.

Please know that due to supply and demand, our cost for these tests have gone up from our supplier. Unfortunately we have to pass this cost onto you. If you can find cheaper tests we encourage you to do so. If you cannot find tests, that is what we are here for.
