A wave of menopause-related legislation has been introduced across the United States this year, leading to possible shifts in how women are cared for during that phase in their lives.

Menopause Update April 2025. In this 30 minute lecture Dr. Donna Shoupe from USC School of Medicine in LA discusses the negligible risk of breast cancer in patients taking HRT. She strongly advocates hormonal therapy for prevention and reduction of Alzheimer's disease, cardiovascular disease, and bone fracture risk. Ladies, this is well worth your 30 minutes. New JAMA August 2024 United Kingdom cohort study of over 117,763 post-menopausal women now links hormone replacement therapy to slower aging process. In other words, everyone has a chronological age and a phenotypic age, measured by 9 biomarkers. This study concludes those women on HRT are associated with lower mortality. Other studies show the route of administration of HRT makes a huge safety difference, with pellets and creams NOT increasing heart attacks, stroke, DVTs, or dementia, compared with ORAL HRT. We also know that estradiol does NOT initiate breast cancer, and over time reduces the incidence for breast cancer formation. Three large prospective studies have shown that a combination of estradiol and testosterone pellets reduce the baseline risk of breast cancer from one patient in 8, to about one patient in 14. Nevertheless all women over age 50 should be surveilled annually for breast cancer with imaging. IMHO, every woman over age 50 needs a consultation with a specialty trained menopause provider at least once to determine if HRT is right for them or not.
