When: Tuesday June 2nd from 5:30-8:00 PM.
Where: Revolution Health-Colon 121 E. Franklin St. Colon, MI 49040

Please arrive with your form filled out and signed by a parent/guardian. If you wear glasses regularly, please bring them for your physical. You can find forms in your school office, RH office or online through MHSAA.
