Please know that if your account has already been turned over to our collection agency you will need to contact them to settle your account.

If you have a balance and want to pay over the phone please call 870-672-0911. I will be here until 4 today. If I don’t answer please leave a voicemail with your name and number and I will call you back.

Please be advised that UCS will be closing and no longer seeing patients. If you are a patient that has any allergy extract, weight loss medications or other injectables stored with us, please make arrangements to pick them up between today and Tuesday. If you would like a copy of your medical records, please call us at the clinic before Tuesday and we will be happy to prepare them for you.

Katy, Leslie, Karri, and Ashton want to thank you from the bottom of our hearts for allowing us to care for you for the past 11 years.
