To all current patients:

This is a notice that Teri Singleterry will no longer be at Sulphur Springs OBGYN Clinic effective October 20, 2025. Your records will remain here with Sulphur Springs OBGYN Clinic unless you give notice that you would like for them to be sent to another provider’s office. If you do decide to transfer your care to another provider, you will need to fill out a release of information and we will gladly send your records to your new provider.
Heather Banker, APRN, CNM will take over the practice effective October 20, 2025, and your care will continue without interruption.
Teri wants to thank each patient that has allowed her to care for them. She is moving on to family practice. She wishes you all the best in the future.
