Did you know? The average adult uses around 200 muscles to take a single step forward.

Much like the individual muscles working together in our bodies, each person on staff at Pathways plays an integral role on our care team.

Pathways fosters a collaborative team of physicians, nurses, home health aides, social workers, therapists (physical, occupational, and speech), dietitians, spiritual care counselors, and grief counselors to meet the growing needs of our patients and families. We are also deeply appreciative to over 150 trained home health and hospice volunteers for their compassionate support and service.

Want to get involved, but don't know where to start? There is something for everyone at Pathways! Visit our Volunteer Opportunities page to learn more https://www.pathwayshealth.org/about-us/volunteers

Impact Spotlight: Culturally Competent Care

Did you see our story about Nurse Honey in our Fall Appeal? That story highlights Pathways’ commitment to providing culturally competent care and empowering our patients to actively participate in their care.

Honey shared, “As an RN case manager, I ask our families about their expectations and knowledge of hospice care. I speak Spanish, and oftentimes I can give Hispanic families a sense of trust when communicating the needs of my client to them in Spanish.”

Patient-centered care is our priority. Learn more about our mission to improve the health of those we serve by visiting our website: https://www.pathwayshealth.org/patient-resources/home-health

Did you know? Walking for 2.5 hours a week can cut your risk of heart disease by 30%. It can also improve your mental and emotional well-being and reduce your risk of depression.

Pathways takes a holistic approach to hospice care, addressing the physical, mental, and emotional needs of our patients and their families. Each hospice patient has an interdisciplinary team of healthcare professionals led by a registered nurse (RN) available to them, including aides for personal care, social workers for counseling, chaplains for spiritual connection, and trained vigil volunteers for support. Hospice from Pathways can be delivered in a private home, a nursing home, an assisted living facility—and sometimes in the hospital.

Our teams who logged the most miles are: Rho Psi Eta, The Foundation Footprints, and CommonSpirit!

Rho Psi Eta shared that many of their teammates are training for long distance runs, like half and full marathons - how awesome!

We still have time to walk and fundraise for meaningful home health and hospice services! Let’s keep up the momentum!

As a dedicated “75-Hard” participant every year, shares these photos from the gym “I love working out. It’s my therapy... It’s funny what our bodies can do - we just have to set our mind to it. Our bodies are capable of anything!”

Keep up the good work, Veronica!

Happy Fun Run Friday!

What do you get when you combine environmental stewardship with exercise? Plogging!

Plogging is a combination of jogging with picking up litter, merging the Swedish verbs plocka upp (pick up) and jogga (jog) gives the new Swedish verb plogga, from which the word plogging derives.

Did you know? Running outdoors at the same pace as on the treadmill burns more energy, purely due to air resistance!

This fun fact shows that location matters! At Pathways Home Health and Hospice, we our mission is to provide high-quality patient and family-centered care in the home, with kindness and respect, promoting comfort, independence, and dignity. Receiving health care at home has been associated with positive outcomes, including lower mortality, reduced hospital readmissions, and functional improvement.
