Covid-19 Vaccination Clinic for ALL VETERANS

When: Saturday, March 27, 2021
9:00 AM until out of vaccines
Where: New Hope Church
1436 GAR Highway, Swansea, MA 0277

IMPORTANT: PLEASE READ BEFORE ATTENDING
• Drive Thru- You do not need to leave your car
• Clinic will be first come first serve
• Now open to Veterans of all ages!
• If you are not enrolled in VA Healthcare, please bring the following:
o A COPY of your DD214 (They will keep it)
o 1010EZ form filled out (Can be found online) https://www.va.gov/find-forms/about-form-10-10ez/
o Picture identification
• Pfizer Covid-19 vaccine being administered. 2nd dose scheduled for April 17th @ 9 AM at the same location.

A new program is going to be available through the state very soon for this population! Please message me here or email elachance@town.swansea.ma.us to be added to the list for eligibility confirmation/appointment scheduling. This is the preferred and fastest method of contact.

It is with great sadness and frustration that I announce our clinics will not be receiving any vaccine for those looking for their first doses this week. Fingers are crossed that we will receive some next week!

For those who have scheduled second doses this week - we have those set aside and will continue those appointments.

240 seniors received their first vaccine yesterday and 2nd doses also began for the first responders vaccinated in January. It’s not over, but at least it’s beginning!

I truly have the most incredible coworkers and volunteers. It takes a village, but we WILL get through this!
