Our office will closing permanently at noon 12/30/2022. You may continue to call 918-708-1535 if you need assistance with obtaining or sending your medical records. Please leave a message and someone will call you back. You may also text that number and it is also our secure fax line. After 1/31/2023 our current fax line will be turned off.
Below we have posted a link to a release that you can download and fax to us to have your records sent to your doctor.
