A breast reduction has one of the highest patient satisfaction rates in plastic surgery. It is life-changing. But after nearly years of performing these, I need to give you the unfiltered truth before you get on the operating table:

1️⃣ I cannot guarantee you a “Full C” cup.
Cup sizes are completely arbitrary and vary wildly from one bra brand to another. My goal is not a specific letter of the alphabet; my goal is to safely remove as much tissue as possible while maintaining blood supply to your ni**le and creating a shape that fits your frame.

2️⃣ You are trading neck and back pain for scars.
There is no such thing as a scarless breast reduction. You will likely have an anchor or lollipop-shaped scar. They fade beautifully over time, but you have to accept that permanent scars are the price you pay to get the weight off your chest.

3️⃣ They are sisters, not twins.
Your breasts were not perfectly symmetrical before surgery, and they will not be perfectly symmetrical after surgery. I will get them as close as humanly possible, but if you are expecting mathematical perfection with a ruler, you will be disappointed.

4️⃣ There is a real chance your ni**le sensation will change.
When we move the ni**le and remove pounds of breast tissue, the nerves get stretched and manipulated. You might wake up with hyper-sensitive ni**les, or you might wake up with zero sensation at all. It is a known risk, and you have to be okay with that possibility.

5️⃣ They can grow back.
A breast reduction does not make your chest immune to biology. If you gain 40 pounds after surgery, go through menopause, or get pregnant, your breasts can absolutely enlarge again. Protect your results by maintaining a stable weight.

A breast reduction will literally take the weight off your shoulders, but you need to go into it with your eyes wide open.

Ever wonder why we look like artists with a marker before surgery? 🎨✍️

Surgical markings aren’t just doodles—they are the absolute blueprint for a successful, beautiful result. Here is exactly why this pre-op step is one of the most critical parts of your entire procedure:

👉Gravity Changes Everything: When you lie down on the operating table, your skin and tissue shift. Marking you while you are awake and standing ensures we account for natural gravity and your normal resting posture.
👉The Ultimate Roadmap: Think of it as an architectural plan. These lines guide every single incision, contour, and lift with millimeter precision.
👉Creating Symmetry: Our bodies are naturally asymmetrical. Markings allow us to measure and map out the exact adjustments needed to create balance and harmony.
👉Safety & Anatomy: We use these lines to map out important anatomical landmarks beneath the skin, keeping your surgery safe and predictable.
👉Our Final Walkthrough: It’s our last chance to review the surgical plan together before you go to sleep, ensuring we are 100% on the same page about your goals.

The surgery happens in the OR, but the vision comes to life during the marking process.

Lipedema is a painful, often misunderstood, and underdiagnosed disease. Surgery to remove this fibrotic fat can give you your life back. But after years of doing this surgery, I need to be brutally honest about what this procedure actually entails:

1. This is not a cosmetic weight-loss procedure❌ The goal of lipedema reduction surgery is to remove diseased, painful fat, restore your mobility, and halt the progression of the disease. While the cosmetic improvement is significant, function and pain relief will always be the priority.

2. You cannot out-surgery an inflammatory diet🥗 Lipedema is highly responsive to inflammation. If you undergo this massive surgery but continue to eat a highly processed, sugar-laden diet, you are sabotaging your own results and risking flare-ups. Food is medicine.

3. You are probably going to have loose skin 👀
When we remove liters of diseased, heavy fat from your thighs or arms, that stretched-out skin doesn’t just snap back—especially if the disease has been progressing for decades and you have advanced stage lipedema. A secondary skin excision surgery (like a thigh lift or arm lift) might be necessary down the road.

4. You can never throw away your compression garments🧦
Surgery removes the lipedema fat, but it is not a magical cure for the underlying lymphatic and vascular issues. Medical-grade compression therapy, dry brushing, and conservative management are still a lifelong commitment.

5. The recovery is an absolute marathon 🏃‍♀️
We are talking about multiple staged surgeries, massive fluid shifts, profound swelling, and significant bruising. It can be physically and mentally exhausting. You need immense patience and an iron-clad support system.

Lipedema surgery is a game-changer, but you have to show up ready to do the hard work before, during, and long after the OR.

Here are 3 high-level questions you need to ask, save this before your consultation:

1️⃣ “How do you prevent the ‘crater’ deformity?”
This is the ultimate test. Some surgeons often aggressively remove all the tissue behind the ni**le, resulting in a collapsed, hollowed-out “saucer” look. An expert knows to carefully leave a precisely measured, small disc of tissue directly behind the ar**la to support it and maintain a natural, flat contour.

2️⃣ “Are you just performing liposuction, or actually excising the gland?”
This is a dealbreaker. True gynecomastia isn’t just fat; it’s dense, fibrous glandular tissue. Liposuction alone will not fix true gynecomastia—it just removes the fat around it, leaving a hard, puffy cone behind. Your surgeon must be skilled in sharp surgical excision to physically remove the gland.

3️⃣ “Can this procedure be performed entirely awake?”
Many patients don’t realize that a massive portion of chest contouring can actually be done under local anesthesia. As someone who specializes in awake body contouring, I can tell you it often means less downtime, avoiding the risks and nausea of general anesthesia, and an incredibly smooth recovery. If they only offer general anesthesia, ask why?

3 Reasons You Must Start Rucking For Lipedema🚶‍♀️

In this video, I reveal how the “gravitostat hypothesis” can help you maintain weight loss by tricking your body’s internal weight sensors. I explain why rucking, or walking with a weighted vest, is a powerful metabolic and lymphatic tool specifically for those managing lipedema. Visit our YouTube channel and watch full video, where you will learn how to safely implement this low-impact exercise to boost your lymphatic pump, increase bone density, and prevent the “rebound” weight gain that occurs when your brain thinks you are starving.

I’m Dr. Markelov, expert lipedema surgeon, Lipedema specialist, and double board certified plastic surgeon specializing in lipedema.

Living in the Sunshine State is amazing, but if you’re recovering from plastic surgery, those intense rays are your scar’s worst enemy☀️🛑

Whether you’re out enjoying the water or just running errands around Tampa, protecting your incisions from UV exposure is absolutely non-negotiable if you want the best possible, barely-there results. Here is why—and exactly how—to protect your healing skin.

🔥 The Danger of the Sun
Fresh incisions are highly sensitive. When exposed to UV rays, your body’s inflammatory response kicks in and ramps up melanin production. This can lead to permanent hyperpigmentation (darkening) of the scar and even cause the tissue to thicken, raise, or widen.

⏱️ The 1-Year Rule
A scar isn’t “done” healing just because the stitches are out. Scars take a full 12 to 18 months to fully mature. Until that scar transitions from pink/red to a flat, faded line, it is incredibly vulnerable to sun damage.

🛡️ Your Florida Scar Care Protocol
1️⃣ Keep it Covered: For the first few months, the best sunblock is a physical block. Wear UPF-rated clothing, wide-brimmed hats, or keep the area under your swimsuit.
2️⃣ Mineral SPF 50+: Once your incisions are fully closed and you are cleared for topicals, apply a broad-spectrum, mineral-based sunscreen (look for zinc oxide or titanium dioxide) every single day.
3️⃣ Beware of “Incidental” Sun: Driving with the windows down, walking across a bright parking lot, or sitting by a sunny window can still expose your healing skin to harmful UV rays.
4️⃣ Silicone is Your Friend: Medical-grade silicone scar tape or gel helps flatten the scar, and the tape provides a great physical barrier against the sun.

Healing is a marathon, not a sprint. Protect your investment and your skin! 🌴

You take amazing care of your face, but what are your hands giving away? 🤫👋

It’s one of the most common frustrations I hear: “My face looks youthful, but my hands make me look older.” As we age, the backs of our hands naturally lose volume. This thinning of the skin and fat leads to a bony, “skeletonized” look where veins and tendons become highly prominent.

The good news? We can fix that beautifully, safely, and naturally 🤍

Introducing the SilkGlove™ Hand Rejuvenation Procedure 🧤— our signature treatment for aging hands. By using a small amount of your body’s own natural fat (usually gently collected from the abdomen or thighs), we can instantly restore that lost volume. This creates a soft, youthful cushion that beautifully camouflages prominent veins and tendons.

Here is why our patients love it:
👉Awake & Comfortable: The entire procedure is done completely awake under local anesthesia.
👉Virtually No Downtime: You can walk out of the office and get right back to your normal daily routine.
👉100% Natural: We are simply using your body’s own tissue to restore what time has taken away.

Your hands do so much for you. It’s time to give them a little love back. 🤍
