Hydroxychloroquine in reduces death rates in COVID-19 when started early. Finally, a well-designed cohort analysis that shows benefit.

Henry Ford Health System has added new drop-off locations for businesses and organizations looking to donate new and unused medical supplies.

The USA has the most confirmed cases in the world because we have performed more tests per capita...more than 500,000 tests completed with about 82,400 positives and 1170 deaths. Real numbers...not China type.

The good news is that the mortality rate is only about 1.4% and may continue to drop as we test more. Meanwhile, 84% of people tested do not have the disease. Since we are mostly testing people with symptoms, the mortality rate could be even lower if you add the folks with little or no symptoms who will probably never be tested. New York alone has 23,000 of the USA cases with 365 deaths (close to a third of all cases and deaths)

We still have a curve to flatten so Stay Home!

***Hydroxychloroquine for COVID-19 – WHY NOT???***

****Update 3/31/2020 - an additional 80 patients added to this French study with similar results**** Also, Dr. Vladmir Zelenko in New York has some interestingly positive findings after treating over 400 patients****

Not a Randomized Controlled Trial and a very small trial but pretty convincing giving we have no better treatment options

Here is my breakdown of this month’s French study:

20 Patients treated – 14 with Hydroxychloroquine only; 6 with Hydroxychloroquine AND Azithromycin (Z-Pak)

At the same time 16 people who were not part of the treatment group were tracked as ‘controls’ for comparison

Results:

14 out of 20 of the treated patients (70%) tested negative by Day 6

All 6 patients (100%) who got Z-Pak in addition to Hydroxychloroquine tested negative by Day 5. By day 4, 5 out of 6 were already negative – 83%)

Only 2 of 16 controls (no treatment) tested negative by day 6 – 12.5%

To put this in perspective, in China, the mean duration of positive testing was about 20 days – longest recorded was 37 days

P-Value at day 6 is 0.001 comparing treated group to untreated group. This means there is only a 0.1% probability that the effectiveness of Hydroxychloroquine in this study occurred by chance

Given the data and the current crisis, WHY should we not be treating every COVID-19 patient with Hydroxychloroquine plus Azithromycin (excluding those in whom it is contraindicated)???

Where? France

When? Early March through March 16, 2020

Who? Patients with COVID-19 infection admitted to French Hospital(s)

Method – non-randomized controlled study

Hypothesis – Chloroquine and Hydroxychloroquine effective against COVID-19 in test tubes in China

I summarized the study by Gautret et al. (2020) Hydroxychloroquine and azithromycin as a treatment of COVID‐19: results of an open‐label non‐randomized clinical trial. International Journal of Antimicrobial Agents – In Press 17 March 2020 – DOI : 10.1016/j.ijantimicag.2020.105949
