The Polk HealthCare Plan is a partnership between Polk County and local healthcare providers that provides coverage for low-income, uninsured residents. The plan is not insurance, and members do not have to pay enrollment fees or monthly premiums.

The Family Healthcare Foundation can help you determine whether you qualify for the plan and apply. To determine your eligibility for the Polk HealthCare Plan, you will need:

* Your valid photo ID
* Your Social Security number
* Documentation showing any sources of income
