To file a claim with UnitedHealthcare (UHC) Choice Plus for reimbursement of your physical therapy treatment with a non-participating provider, follow these steps:

1. Sign in to your UHC member account:
* Go to the UHC website and sign in to your member account.
* Navigate to the “Claims & Accounts” tab, then select the “Submit a Claim” tab.

2. Download the Medical Claims Submission form:
* You can download and print the Medical Claims Submission form from the “Submit a Claim” section.

3. Complete the form:
* Fill out all the required fields on the form. You may need to ask your physical therapist for their provider information or have them fill out their section of the form.

4. Attach the necessary documents:
* Attach the itemized statement or a fully legible copy of the bill from your physical therapist.
* Keep a copy of the form, claim details, and receipts for your records.

5. Submit the claim:
* Mail the completed form along with the claim details and receipts to the address provided on your health plan ID card1.

~After submitting your claim, UHC will process it and send you an Explanation of Benefits (EOB) detailing the charges applied to your deductible and any amounts you may owe the provider
