Services Provided: The most widely used model of outpatient physical therapy care provided today is within a facility that accepts your insurance. In most cases, when utilizing your insurance for physical therapy, due to the ever-reducing reimbursement rates, physical therapists are left with no choice but to see multiple patients per hour and utilize physical therapy technicians to help facilitate your care. Often times, the physical therapist is unable to spend more than 15-20 minutes with each patient and the patient is not receiving the full benefit of what a physical therapist can prescribe. Returning to your previous health or preferred level of function is something that is personal and unique to you. I believe that insurance should not dictate your goals when seeking optimal health and you deserve to receive the full benefit of the education and treatment I am able to provide. The treatment I provide is one-on-one and is focused on manual therapy, patient education and specific, very detailed exercise instruction with the ultimate goal of returning each patient to their prior level of function or quality of life as soon as possible (which is often much quicker than with the utilization of the insurance based model). Each session is approximately 60-75 minutes in length and is encompassing of a thorough assessment of your function at each visit with hands-on/manual therapy, as needed, and a personal exercise program specific to your needs. You also have access to me via e-mail and phone/text, as necessary. Cost: Due to the extensive and detailed nature of each one-on-one session, the initial evaluation cost is $175 and each follow-up visit is $125. Foundation Physical Therapy does accept Health Savings Accounts and Flexible Spending Cards. You will also be provided with a detailed invoice of the services provided, which may be utilized to submit for reimbursement for the services you received under your “Out Of Network Benefit.”
Access to Physical Therapy: Arizona is considered a direct access state for physical therapy. This means that you are able to directly refer yourself for physical therapy without a physician’s referral. However, shall you prefer to seek the care of a physician prior to your appointment, or if after your initial exam I feel a physician appointment would be necessary, I can provide you with a referral. Please do not hesitate to contact me with additional questions or concerns. Renee Hodges, DPT, ITCA Triathlon Coach

Foundation Physical Therapy and Endurance Coaching was founded by Renee Hodges. Renee received her Bachelors Degree in Biology from Augustana College in 2000 and her Doctor of Physical Therapy from AT Still University in 2003. Renee began her Physical Therapy career 17 years ago providing care in the orthopedic setting where she developed a love for treating sport related injuries. Over the last ten years, Renee has developed a niche practice of diagnosing and rehabilitating endurance sport related injuries. Renee’s assessment of endurance athletes is focused on an in-depth review of the patient’s athletic history, along with a detailed exam of the patient’s functional movements and endurance strength tolerance for all repetitive movement patterns required for their sport. She also performs a through video analysis of running mechanics for current assessment and future comparisons. Each athlete’s treatment plan reflects a strong emphasis on developing the correct neuromuscular pathways for producing proper movement and endurance strength for the athlete’s sport. Each treatment builds upon the next and is a progressive plan to gradually return the athlete to their activities under a slowly guided and carefully watched program. Renee is very passionate about her athletes as she is an active member of the same local endurance community with which her patient’s participate. She regularly competes in trail races, half marathons, marathons and triathlons of varying distances. She ran the Boston Marathon in 2011 and completed Ironman Arizona in 2011 and 2013 (placing 8th in her age group).
