A-Plus Home Health Services has been a leading provider in delivering quality home health services to patients throughout the East Valley. We provide patients with an easy and effective way to communicate with our medical staff as we work to keep them happy and healthy in the comfort of their own home. We are a family owned and operated business that service the geriatric population here in The East Valley. We provide In-Home Skilled Care to Medicare and BCBS recipients that are typically 65 + and are homebound. Our dedicated staff includes; registered nurses, physical, occupational and speech therapists, medical social workers, dieticians and home health aides. They are all committed to providing our patients quality healthcare in home. Below are some services we provide @ A-Plus HHS:
◾Nursing - We have Registered Nurses (RN) that can visit patients at scheduled times to provide nursing services, observe and evaluate their progress and make regular reports to their respected physician.The nurse makes special efforts to include both the patient and family for their care and overall healing process.
◾Physical and Occupational Therapists - Our certified therapists work with patients to help them regain strength, endurance and motor skills that have been lost due to an accident, injury, stroke or illness.
◾Speech Pathologist - Our certified speech pathologists are available to work with patients who have speech impairments, hearing problems or swallowing disorders.
◾Medical Social Worker - Our Medical Social Workers offer counseling for long-range planning and decision-making, assistance in evaluating patient and family social and emotional needs and help in coordinating community services such as Medicaid, meals, companionship and chores.
◾Dietician / Nutritionist - Individuals with certain medical conditions may need to follow a specific diet for the rest of their lives. A-Plus HHS can provide a dietician / nutritionist that can help provide education and information on a variety of diets, including low-cholesterol, cardiac, renal, low salt and diabetic. The nutritionist helps patients and families understand how to adjust to their new way of life.
◾Home Health Aide - Our Home Health Aides assist patients with all aspects of daily living, including dressing, grooming, meal preparation and transfers (i.e. from bed to chair).
