Sports physicals at CHCS are a quick, affordable way to make sure your child is healthy and safe to compete. Our providers check heart health, blood pressure, vision, musculoskeletal function, and more โ€” everything required by most school athletic programs.

Don't wait until the week before tryouts. Getting ahead of the rush means fewer scheduling headaches for you and a faster clearance for your athlete.

Community Health Care Systems, Inc. is a non-profit organization dedicated to providing high quality care that includes primary care and behavioral health services to individuals and families. In addition, CHCS also provides a 340(b) pharmacy, pharmacy delivery. podiatry, gynecology, telehealth, chronic care management, health education, disease prevention programs, community outreach, and enrollment services. We accept patients without insurance as well as patients with insurance. We accept all forms of insurance and strongly believe our clients have the right to quality care that meets their needs regardless of ability to pay.
