Congratulations Grad!

Did you know patients can graduate from hospice services? Hospice graduates are patients whose conditions have improved to the point where they no longer require hospice for the time being. Although it’s highly unusual for a hospice patient to recover from a terminal illness; with 24/7 support and symptom management their condition can stabilize.

As a team we were able to manage Bayard’s symptoms so much so that he wanted to try therapy. Now he has transitioned from the skilled nursing facility to the assisted living facility and is independently caring for himself!

Our patients are one in a MELON and we are so fortunate to have the opportunity to care for them!

One of our patients, Marvin, wanted to have a picnic lunch with his clinical team and specifically requested watermelon. Our Executive HCC, Candi, was able to organize a beautiful gathering.

Our Hospice Care Consultants Amanda and Madison hosted a Lunch ‘n Learn with Hendricks Family Medicine.

We were able to educate on the indicators of eligibility as well as provide a map of our service area. This was reassuring to the providers as they serve a rural area where many of their patients drive over a half-hour.

Our hospice care consultants are out celebrating our independence and educating on our We Honor Veterans program.

Our We Honor Veterans program, in collaboration with the Department of Veterans Affairs (VA), empowers end-of-life care partners to meet the unique needs of America’s veterans and their families.

We are proud to be a Level 4 branch and have committed to offering veteran-specific care and services. Through continued education and training, we are better equipped clinically to care for our veteran patients, who have endured various conditions/circumstances that need our dedicated attention at the end of life.
