“We Remember Them”
for the hearts of Concho Hearts Hospice
At Concho Hearts, we walk the line
Between the fading and divine.
With every breath, in every home,
We remind them they’re not alone.
The nurses come with healing light,
Through pain’s long shadow, they ignite
The comfort found in touch and tone,
A voice that says, “You’re not alone.”
The CNAs, with hearts so wide,
Bring dignity, and quiet pride.
They lift with love, not just with hands,
Like angels walking through the land.
Social workers, strong and kind,
Listen close, and help them find
The strength to speak, the will to cope,
The buried roots of fading hope.
And chaplains sit with holy grace,
In silence deep, they find their place
A whispered prayer, a knowing glance,
A soul embraced in one last dance.
But still, we carry what we hide
The tears that come when someone’s died.
We cry for them like family do,
Because to us, they were that too.
We know their smiles, their laughs, their songs,
The way they fought to stay so strong.
The trembling hands, the stories shared,
The way they looked when someone cared.
And yet, some rise, survive, outgrow
The path we thought they had to go.
Graduates of a different grace,
Who leave our care with smiling face.
We cheer for them, we hold it dear,
Those rare bright moments every year.
Because at Concho Hearts we see
That hospice holds life’s mystery.
Not always endings, sometimes light.
A second chance. A newfound fight.
Our work is love, our mission true
To bring the best of life to you.
So when you ask just what we do,
We guide the soul, we journey through.
We comfort pain, we celebrate,
We grieve, we heal, we advocate.
We hold their hands, and sometimes more
We hold their lives within our core.
At Concho Hearts, we give our best
And carry each soul in our chest.
