Interstitial Cystitis (IC) — also called Bladder Pain Syndrome (BPS) — is a chronic condition that causes bladder pressure, pain, and frequent or urgent urination, without an infection. It can range from mild discomfort to severe, daily pelvic pain.

Key Facts:
🔹 Not caused by infection, but often feels like a UTI
🔹 More common in women, but can affect all genders
🔹 Symptoms can come and go (flares & remissions)
🔹 Often overlaps with pelvic floor dysfunction, IBS, endometriosis, or fibromyalgia

Common Symptoms:
🛑 Pain in the bladder, pelvis, or urethra
⏱️ Feeling the need to p*e constantly, even if the bladder is not full
🚽 Waking up multiple times at night to urinate (nocturia)
🔥 Pain during or after urination
🧡 Pain during or after sexual activity

Many people with IC have tight, overactive pelvic floor muscles. Physical therapy helps by:

Let’s talk about the real core:

May is mental health awareness month!

So much of Pelvic Floor Physical Therapy is physical, but there is much that can impact the pelvic floor outside of the body. The brain-body connection is incredibly influential in your body's responses and, although we are experts at addressing the physical, we need both to be healthy for the greatest success. This is where working together with a mental health specialist is a game-changer!

Pelvic PT often includes exercises, such as diaphragmatic breathing and pelvic floor relaxation, to enhance blood flow to the pelvic area, which can increase natural lubrication and improve tissue health.

🧘 Relaxation Techniques for Muscle Tension
Pelvic floor muscles can become tense or overactive due to stress, pain, or trauma, reducing blood flow and lubrication. Pelvic PT uses manual therapy, biofeedback, and relaxation techniques to release muscle tension, improving tissue elasticity and comfort.

⚡️ Addressing Pain and Discomfort

Vaginal and re**al dilators are medical devices used for therapeutic purposes to gently stretch, expand, or maintain the flexibility of the vaginal or re**al tissues. Here’s a breakdown of their uses:

Vaginal Dilators-

1. Vaginal Stenosis: To prevent or treat narrowing of the vaginal ca**l, often due to radiation therapy or surgery.
2. Vaginismus: To help with involuntary muscle spasms that cause pain or discomfort during vaginal pe*******on.
3. Post-Surgical Recovery: To prevent scar tissue formation after surgeries like vaginoplasty or other gynecological procedures.
4. Gender-Affirming Care: Used post-vaginoplasty to maintain the depth and width of the neovagina.
5. Pelvic Floor Dysfunction: Helps relax the pelvic floor muscles when they are overly tight or in spasm.
6. Post-Menopausal Vaginal Atrophy: To stretch the tissues and make vaginal exams or in*******se more comfortable.

Re**al Dilators-

1. A**l or Re**al Stenosis: To widen a narrow re**um or a**s, which may result from surgery, trauma, or certain diseases (e.g., Crohn’s disease).
2. Anismus: A condition where the muscles around the a**s do not relax properly during a bowel movement, leading to constipation. Dilators can help train the muscles to relax.

What we want:
😌 Manage Pain and Improve Sensation
🎯 Goal: Reduce pain, swelling, and discomfort. Improve tissue feeling
and sensation in abdomen and along scar.
🧊 Modalities: Heat, cold packs, desensitization with varying textures,
and gentle touch/massage.
🧍‍♀️ Postural and Movement Training
🎯 Goal: Improve posture and movements impacted by pregnancy and
surgery.
🛏️ Education: Guidance on safe lifting, getting out of bed, sitting and
holding baby, breast/bottle feeding and performing daily tasks
without abdominal strain.
🌬️ Breathing Exercises
🎯 Goal: Restore normal diaphragmatic breathing and promote
relaxation.
🧘 Techniques: Diaphragmatic breathing to engage the core, coordinate
with pelvic floor muscles, and enhance oxygenation.
🧴 Scar Tissue Mobilization
🎯 Goal: Prevent excessive scar tissue, improve flexibility, and reduce
adhesions.
🤲 Techniques: Scar massage, stretching, and tissue mobilization
exercises.
💪 Core and Pelvic Floor Strengthening Exercises
🎯 Goal: Restore strength to deep abdominal muscles and pelvic floor
muscles postpartum and post-surgery.
💪Exercises: Individualized but can include pelvic tilts, bridges,
quadruped, kegel training, etc.
🚶‍♀️ Gradual Return to Activity
🎯 Goal: Safely return to normal daily activities and exercise.
🏃‍♀️ Program: Gradual increase in walking (to running if desired),
resistance training, and functional movements.

Round ligament pain is common during pregnancy. The round ligaments are bands of connective tissue that support the uterus. As the uterus expands during pregnancy, these ligaments stretch, which can lead to sharp or stabbing pain, usually in the lower abdomen or groin area.

Tips to Relieve Round Ligament Pain:

✨️Change Positions Slowly: Move gently and consciously while engaging your core (talk with a pelvic PT on how to do this CORRECTLY) and avoid sudden movements.

✨️Rest: Lie down on your side, typically on the side opposite of the pain.

✨️Support: Use a maternity belt or a pillow to support your abdomen.

✨️Warmth: Applying a warm (not hot) compress to the area may help soothe the pain.

✨️Stretching/strengthening: Gentle stretching exercises, prenatal yoga, and appropriate strength training can support the ligament and alleviate discomfort.

Breastfeeding can have many challenges, one of which is protecting your posture to avoid pain or symptoms. As physical therapists, we help manage postural challenges and keep you functional while caring for your baby.

Here are some tips we commonly discuss with breastfeeding parents. Experiment with different positions to find the one that suits you and your baby best.

And reach out if you are having any musculoskeletal symptoms related to posture or pelvic floor while breastfeeding or postpartum!
