Just a kindly reminder to those coming to us for Fed Ex or Formfox DOTs please call our clinic to confirm appts or to set up appts. Just cause FedEx or whatever company set up your appt does not mean they spoke directly to us. Please take time to call and confirm your appt is set up. Please do not be rude to Staff about your mistake on not calling to confirm. We do not accept Walk-Ins for DoT physicals appointment are required for these type of appointments. Thank you.

We are accepting new patients we take most insurances INCLUDING Medicaid(only Medicaid we do not accept currently is BCBS).

Give us a call if you have any questions. 217-703-1359. Ask for Shane or Celia and we can answer any questions you may have.

We also do DOT physicals $100 for the physical.

Hours varies daily due to the providers schedule but we can accommodate most for scheduling.

An update on Steve, he just he will be out of the clinic till Monday we will be calling the people we have scheduled tomorrow to move the schedule around thank you and sorry for the inconvenience.

The clinic will be closed today due to Steve being out sick. We will resume our normal hours tomorrow if you need an appt feel free to call and leave a voicemail we will return your call the next day thank you.

This is a heads up for the week of Christmas on to the new year the clinic will be closed from 12/24-1/1 We will reopen on 1/2/26 for half day and then clinic will resume normal hours on 1/5/26 of you are needing refills or needing appointments please call and leave voicemails Shane will going into the clinic every other day to check for messages. For anyone that comes to our clinic and smokes for now on ci******es are not allowed on the property....we just had to put out a fire that melted our front drain pipe...thanks for who ever flicked there cigarette and did not take the time to put it out...

Just a friendly reminder to everyone the office will be closed from July 28th- August 1st. Steve will be on Vacation during this time so if any medication need refilled during this time frame it will have to wait until he returns to the office on August 4th.
