* Provide individual and group therapy for clients with substance use and co-occurring mental health disorders
* Develop and implement individualized treatment plans
* Complete clinical documentation in a timely and compliant manner
* Collaborate with a multidisciplinary team to support client success
* Help create a safe, engaging, and supportive therapeutic environment

* Licensed in Ohio (LSW, LISW, LPC, LPCC or equivalent)
* Experience in substance use disorder and/or mental health treatment preferred

Attain Behavioral Health is seeking a compassionate, motivated, and skilled Behavioral Health Engagement Specialist (LSW) to join our growing team! This role focuses on providing behavioral health support within a nursing home setting, helping residents improve their mental health, emotional well-being, and overall quality of life.

Position Overview

As a Behavioral Health Engagement Specialist, you will work directly with residents in long-term care facilities to assess needs, provide therapeutic support, and coordinate care. This is a unique opportunity to make a meaningful impact with a population that often goes underserved.

Key Responsibilities

* Provide behavioral health assessments and ongoing support to nursing home residents
* Deliver individual and group interventions focused on mental health and substance use
* Collaborate with nursing home staff, physicians, and interdisciplinary teams
* Develop and implement individualized care plans
* Monitor client progress and adjust services as needed
* Maintain accurate and timely documentation
* Serve as a supportive resource for residents and their families

Qualifications

* Licensed Social Worker (LSW) in the state of Ohio (or eligible)
* Experience in behavioral health, substance use, or geriatric populations preferred
* Strong communication and interpersonal skills
* Ability to work independently while collaborating with a team
* Passion for helping others and making a difference in the community

Compensation & Benefits

* 💰 Salary: $60,000 – $75,000 (based on experience)
