Foundation Park Alzheimer’s Care Center
OUR HISTORY~Foundation Park Alzheimer’s Care Center was established in 1989 as the first free standing center in Northwest Ohio specializing in the treatment of Alzheimer’s disease and related dementias. The center is fully licensed by the State of Ohio Department of Health to serve NCM’s principals and its management team have an impressive track record in developing and managing healthcare facilities and services. James L. Farley and Michael J. Scharfenberger, NCM executive officers, offer more than 50 years of combined experience in developing and managing long term care facilities. Both are licensed healthcare administrators and are Fellows of The American College of Health Care Administrators. NCM was established in 1984 with a commitment to excellence and a clear vision of the future of the dynamic healthcare industry. The expertise and foresight of the management team, as well as the progressive management systems they have designed and implemented in their facilities, have earned NCM a national reputation for professionalism and quality in healthcare management. NCM has grown from a single nursing home in Cincinnati, to a network of owned and managed facilities in several cities throughout Ohio and other states. Foundation Park Alzheimer’s Care Center opened it’s doors on March 23, 1989. Foundation Park is a Corporation unto itself and stands on its own. Its legal name ndis Tlevay, Inc. d.b.a. Foundation Park Care Center. Foundation Park Alzheimer Care Center is the first free standing Alzheimer’s facility in Northwest Ohio.
