Your Healthcare Choices: Connecting Through Conversations At any age, a medical condition could leave you too ill to make healthcare decisions. This page will We never know what may happen to us on any given day.

Advance Care Planning is the process of learning about the types of medical care that you may or may not want. If you are ever in a situation that may leave you too ill to make medical decisions for yourself, it’s important for you to have a conversation about what your wishes are while you are able to do so. Part of this process includes creating an advance medical directive which allows you to name someone to speak for you if you become too ill to make them yourself. EVERYONE is encouraged to complete an advance medical directive and periodically update them throughout your lifetime. I am a mother of two young children and have been an ICU nurse for the past 12 years. I have held many hands and wiped many tears of family members and loved one who were asked to make very difficult decisions without ever having a conversations about what their loved one would want. I have created this page to provide you with some great resources, videos, educational materials, and conversation tools to encourage you to start this important process of advance care planning. Give your loved ones this gift of know what your wishes are, and take the burden off their shoulders. Please share your stories, ask questions, and let us learn from each other.
