Dear Parents and Guardians,

Child Health Associates prides itself on providing excellent, up-to-date, evidence-based care. We know that the COVID-19 vaccine landscape in children has become more complex, and there is a lack of agreement between the agencies that we usually use for guidance.

The Center for Disease Control (CDC) and American Academy of Pediatrics (AAP) each have their own guidelines. If you would like the details of their policies, they can be found at the links below:

We understand that this is confusing and
recommendations are evolving. Our views are in line with the AAP recommendations and we will continue to provide the COVID-19 vaccine to those who are interested in vaccination.
For ages 6 months - 4 years, we will be offering the Moderna COVID-19 vaccine.
For ages 5-11 years, we will be offering the Pfizer vaccine.

For ages 12 and older, please go to your local pharmacy.

Before we administer the vaccine, we will provide you with an Advance Beneficiary Notice (ABN) form explaining the uncertain regulatory and reimbursement environment.
Your insurance carrier may not provide coverage, and if it does not, you will be responsible for the cost. The estimated cost for 6 months - 4 years will be $125 and for ages 5-11 years will be $145.
