What Is Angiography ?

Angiography is a diagnostic testing procedure performed in the office to help a retina specialist diagnose disease, further classify pathology, help guide treatment (ie intravitreal injections, PDT, laser photocoagulation), and follow the course of your condition. Fluorescein dye is injected into a vein in the arm (very similar to a blood draw) and travels into the retinal circulation where specialized cameras are used to track its course through arteries, veins, and capillaries

Sudden blurring or loss of vision may occur in:

All of one eye (central retinal artery occlusion or CRAO)
Part of one eye (branch retinal artery occlusion or BRAO)
The retinal artery occlusion may last for only a few seconds or minutes, or it may be permanent.

A blood clot in the eye may be a warning sign of clots elsewhere. A clot in the brain may cause a stroke.

What Is a Posterior Vitreous Detachment?

The vitreous is the gel-like substance that fills the back of the eye. When you are young, this gel is quite firm with a uniform consistency and is adherent to the surface of the retina. As you get older, the gel begins to break down and develop pockets of liquefaction. Within these pockets, the gel proteins clump together which lead to the experience of floaters. In most people, at some point in your life, there is sufficient breakdown of the gel that it begins to separate from the surface of the retina. If the separation occurs gradually, most patients will be unaware of the changes. If there is a sudden collapse of the gel off the retinal surface light flashes or the sudden onset of floaters can develop. Where the posterior vitreous is attached to the optic nerve at the back of the eye, is a point of more firm attachment, and when this part separates, a ring of condensed vitreous gel which previously had been the site of attachment to the optic nerve, "floats" in the middle cavity or of the eye. Often this ring is seen a gray, white or black doughnut if seen dead on, or as a squiggly line if seen from the side. If the gel does not separate cleanly or pulls too strongly on areas in the peripheral retina or if preexisting weak spots are present in the retina e.g. lattice degeneration, then tears can develop.

Lattice degeneration, or simply "lattice" is a degeneration or thinning of the peripheral retina. Although it can come in many different variations, a typical appearance looks like the lattice crust on a fruit pie a straight, linear, and criss crossed pattern. Lattice is very common, occurring in about 8% of the population. When present in one eye, 50% of the time it is present in the other eye as well.

Branch retinal vein occlusion is a condition in which part of the venous drainage system of the retina is blocked. The retinal artery supplies blood to the retina. The blood flows through retinal arterioles, capillaries and finally through branch retinal veins that drain into the central retinal vein. A blockage in one of these branch retinal veins causes back pressure and leads to hemorrhage, exudation (leakage of fluid), and/or decreased blood flow in the area of the retina drained by that particular branch retinal vein.

There are many inherited retinal disorders. Some are severe and may cause blindness, whereas others may remain asymptomatic though an entire patient's life. They run in families (familial or inherited) and may be autosomal dominant (strong inheritance that does not skip any generations) or autosomal recessive (less strong inheritance that may skip many generations). What Are the Causes of Retinal Artery Occlusion?

Retinal arteries may become blocked when a blood clot or fat deposits get stuck in the arteries. These blockages are more likely if there is hardening of the arteries (atherosclerosis) in the eye.
Clots may travel from other parts of the body and block an artery in the retina. The most common sources of clots are the heart and carotid artery in the neck.

Most blockages occur in people with conditions such as:

- Carotid artery disease, in which the two large blood vessels in the neck become narrowed or blocked.
- Diabetes
- Heart rhythm problem (atrial fibrillation).
- Heart valve problem.
- High levels of fat in the blood (hyperlipidemia).
- High blood pressure.
- Intravenous drug abuse.
- Temporal arteritis (damage to arteries due to an immune response).

Who Gets Uveitis?

Uveitis may be caused by a traumatic injury to the eye, or by infection inside the eye (e.g. herpes or tuberculosis), but most commonly uveitis is caused by an autoimmune process. Autoimmunity is a disease process where the body misidentifies part of itself and "attacks" this misidentified part as foreign. The "attack" takes the form of white blood cells and inflammatory substances which react as if the tissue they are attacking doesn't belong and needs to be removed or walled-off. The uvea is a part of the body more frequently misidentified in this way than other parts of the body. Some autoimmune diseases affect only the eye (e.g. pars planitis and birdshot chorioretinitis). Others affect the eye and may or may not affect other parts of the body at the same time.

The object of any retinal detachment repair is to get rid of the fluid underneath the retina and allow the retina to lay "flat" upon the concave contour of the wall of the eye. There are several methods of achieving this goal, each of which will be discussed in the sections below. The great majority of retinal detachments are of the rhegmatogenous type, and these sections will primarily discuss repair of this type of detachment. At the end of these sections, there will be some more specific comments about traction and serous retinal detachments.

How Are Retinal Tears Treated?

All tears that are new or any tear associated with sudden new symptoms of flashes and or floaters require prompt treatment. The object of treatment for a retinal tear is to seal the edges of the tear to prevent fluid from passing through the tear and extending beneath the retina leading to detachment. The usual way such an adhesion is formed is by creating a circle of small scars around the tear with a laser. Sometimes if the view of the retina is so poor, i.e. from a dense cataract or from blood in the vitreous cavity, the adhesion is created by performing freezing treatment (called cryopexy) from the outside of the eye with a cryoprobe.
