UPDATE: 12/8/24-9PM

Good evening everyone I wanted to jump on and let you all know that there really hasn’t been much change the nurse told me that he had a somewhat restless night but was sleeping and seemed comfortable. They are suctioning out mucus from his mouth But that is expected.

I will post when I have new information or something changes.

UPDATE: 12/08/24-12:30 am.
Good evening everyone! I wanted to share a picture of my dad. this picture was taken earlier today about an hour before he was transferred to the hospice.

The transfer went very well! He does not like being on oxygen so we decided to go ahead and remove it. His vitals are actually the best we have seen so far.

What’s the plan now?

The plan going forward is to keep him comfortable. The doctor ordered quite a few comfort medications. He no longer has any scheduled medications. All will be as needed.

I will post the contact information in the morning.

As well as any updates I have. Or if anything changes.

UPDATE: 12/07/24 -8:38pm

Hello everyone I want to come and provide a quick update. I just signed the hospice paperwork he is scheduled to be moved at 10pm local time. They are removing the high flow oxygen now as well as all IV’s.

What does this mean?

Well this means that he will still be on 15 liters of oxygen but that will go away over time. He is now on a comfort care plan. With this he will get comfort meds and that is all.

How long does he have?

Well that honestly is something none of us know. Doctor believes it will be pretty fast but again it is hard to say.

Where is the hospice home at?

I will post all the details to the hospice home in the morning. As tonight is going to be transitional time for him alot is happening and I have to establish permissions for the hospice home.

What can I do?

UPDATE : 12/7/24 at 6:13 pm -Good evening everyone . I just spoke with the doctor today at around 12:30pm. The doctor informed me that though his vitals are good when the oxygen is lowered he does not respond well at all. He is current on 37 liters of oxygen.

What does this mean?

This means that life is not viable with out oxygen. This also means that like I said before we are now looking at quality of life, as well as his current condition.

What’s next?

Currently the pan is to meet with hospice and get him moved to over there. This is going to to happen tonight. I don’t know if they will move him but it is likely.

How long does he have?

Once he is placed on comfort care his oxygen will be reduced to 15 liters and slowly decreased to zero. Once that is gone he will be given comfort medication. It is expected he will pass very quickly after. The doctor gave him 24 to 48 hours. It could be longer could be shorter.

What happens after?

He will be transported to the funeral home and cremated. All of this is payed for already.

Is there a service planed?

No there is not. My dad asked there not be a formal service but a BBQ to celebrate him. I am okay if someone in the family wants to pay for a spot a cemetery I will support this but I can not pay for this nor can my dad. This is not what he asked for so it is not something that was planned.

UPDATE: 12/6/24 -10:15pm….hello everyone! I just spoke with the nurse at the hospital and my dad is more awake his vitals are stabilizing more.But the nurse did let me know that he is having difficulty swallowing and is not eating or drinking because of this. Both the nurses that I spoke with let me know they are having to tether his hand because he is trying to pull IV ‘s and all the equipment off of him.

What does this mean?

Well this is not good. This is a situation where the possibility of a feeding tube will probably come to the table. The issue with this is he will more than likely attempt and probably successfully pull this tube out. The danger is if he pulls on this tube and it is not noticed it can begin putting ground up food in his lungs. (**Side note** the pneumonia was caused by him not properly chewing and swallowing his food. ) he can also severely damage his airway if he successfully pulls it out.

Where do we go from here?

Well that is yet to be determined. There are several factors that come to play in this situation.

1. Quality of life
2. Safely
3. Is this something that is temporary or permanent

Now let me explain these points. The first and foremost thing we have to consider is if we do this will this improve his quality of life now and in the future. Meaning how will this affect him right now and how will this affect him down the road.

The next point is safety. Is a feeding tube safe for him? The short answer is no. Now let me explain. The type that they will most likely do is a NG tube(through the nose) these are meant to be temporary. The second option is a PEG tube( through the stomach) this one is more of a permanent option. The issue with the PEG is he has to be lightly sedated to do it and it requires a small surgical procedure. Now in a normal situation this is not a big deal. But in this situation the care team has to consider safety and quality of life and how does this affect the DNR. this is the part I am having to really look at as this is a legal document I will not go against as he filled it out (this is what he wanted) Currently the concern is he would probably need to have the PEG tube. Due to the fact it would probably be permanent. The issue with this is surgery is it is very risky and he may not recover fully from it(due to exsisting medical conditions). The other issue is he will more than likely pull it 5 minutes after waking up and that opens the possibility of infection. The second question quality of life. Well this a very difficult one. When determining this one this is what I am looking at. I know my dad and I know he loves food and loves to eat with the other residents at the care home. If he has this in permanently (likely the case) he would not be able to eat or drink Normally. This would cause immense frustration and likely cause his to want to pull it out more.

Finally is it permanent or temporary?

Well we don’t know yet. This is yet to be determined. I will say the nurse feels it maybe permanent and agreed that the three points are very valid above.

What can I do to help?

PRAY! PRAY!

How will you make this decision?

Well that is a tough one. I am going to be meeting with a social worker and his medical team tomorrow to determine what is next. We will explore all options and evaluate what is the most ethical option. I personally am going to be praying asking the lord to guide me through this.

Can we have a say in what happens?

YES! I am open to hearing your input. If you wish to have a meaningful discussion please feel free to message me. I would love nothing more than to not have to make a decision without knowing the whole family’s thoughts. I value everyone’s opinion and will take into account everyone who wishes to share. I just ask that it is a constructive conversation.

UPDATE: 12/06/24 Hello everyone, I want to provide an update as of 6pm this evening. He is stable, and his vitals are stable. He responded well to the fluids given last night. He has pneumonia, which was caught early. According to the doctor, it is very treatable, but his health history means it can take a turn for the worse quickly. To prevent this, he is on strong antibiotics. He has been moved out of the emergency department and into a step-down unit, which is a critical care unit, just below ICU. They did test for sepsis and initial tests came back negative, they also did blood cultures and are letting them sit for 24/48 hours and will look for bacteria in the blood if they find bacteria in his blood that means he is septic. This is the worst case scenario for him as that along with pneumonia can be fatal. They are hoping the initial test was right but said that because his sodium levels were elevated they suspect it may be wrong.

What are they doing to try and counter this:
Well the answer is actually very complicated. The reason it is complicated is because he has a DNR (Do not resuscitate) so what this means is the doctors can not do normal care. They are only able to treat with medication they can not do advanced life support or ALS. So they are using some of the strongest medications they can to fight the pneumonia but it is typically treated by intubation and other medications. The doctor seemed pretty confident they could treat this without using the ALS techniques.

What does all this mean?
Well we have done everything we are legally allowed to do without removing the DNR.

What happens next?
We wait and hope this treatment works.

What am I doing as DPOA:
I have instructed the hospital to do everything they can to treat it within the guidelines of his DNR. You may wonder why this is in place. This was put in place in march of 2022 at my dad’s request. We met with the doctors and went over what it means and what he is okay with in regards to how we handle this situation. He asked that he be taken to the hospital but no life saving messures be taken. He further specified that no CPR or chest compressions be done. After this was completed in march of 2022. I went to his care team some time later and asked if this sort of document was needed as he was doing fairly well and didn’t seem to be showing signs of decline. His whole care team agreed that it was very necessary and should stay in place.

How can you help:
honestly just pray for him. Please feel free to ask questions! I am more than willing to hear what my other family members have to say. Please know that any and all decisions made during this time and any other times are made based off what the doctors are recommending we’ll also looking at the bigger picture of how it will affect quality of life as well as other factors. I do not take this responsibility lightly and I am doing everything I can to ensure my dad gets the best care possible. I ask for everyone to be respectful and understand that I am following the advice of his care team and the legal documents in place.

Yes you can visit he is at Tucson medical center. Just go to the front of the hospital and they will direct you to his room. You can also call Tucson medical center and ask to speak to his nurse.
