Faith Home Care is a growing, Oklahoma-owned and based, home health company that is seeking a
Full-Time Registered Nurse Case Manager.
Faith Home Care is seeking an outstanding RN that has compassion and drive and strives to provide the best care possible for their patients. The RN will be responsible for Case Management, performing Start of Care, Re-certifications, Resumption of Care, Discharges, Transfers, and Skilled Nursing Visits as scheduled/needed.
Qualifications/Must Have:
Current Oklahoma Registered Nurse License
Valid Drivers License
Cell phone
Current CPR
Great bedside manner, as well as phone and written communication skills
Reliable transportation
A love for Nursing
Be a team player

Compensation is Salary with on-call pay and mileage reimbursement.
$125/weekend on-call plus per visit rate. On Call is rotated between nurses
