We are now offering Chronic Care Management (CCM) services for Medicare patients. CCM is available to any Medicare patient that has 2 or more chronic conditions. Benefits of CCM include better access to care and improved outcomes for patients including reducing hospitalizations.

Our CCM Nurse is KC and she is located in our office. She has worked at TMG for over 5 years and was previously Mrs. Carmen Raines nurse.
