Families Together of Orange County has been named a 2025 National Quality Leader for Diabetes Care by HRSA — one of the highest recognitions a community health center can receive!

This achievement reflects the hard work and dedication of our entire team, who came together to make sure patients living with or at risk of diabetes received comprehensive, coordinated care — from vision screenings and podiatry services to preventive education. 💙
