Our care team works together focusing on the patient's needs in their home, home of a loved one, or in a long term care or assisted living facility. Physician services for the medical direction of the patient’s care
Regular home visits by registered nurses and licensed vocational nurses and hospice aides
Social work and chaplain visits to assist with final arrangements
Bereavement support
Medical equipment such as hospital beds and oxygen
Medical supplies
Medications for symptom control and pain relief
Volunteer support to assist patients and loved ones
Veteran services
The We Honor Veterans campaign provides tiered recognition to organizations that demonstrate a systematic commitment to improving care for Veterans. “Partners” can assess their ability to serve Veterans and, using resources provided as part of the campaign, integrate best practices for providing end-of-life care to Veterans into their organization. By recognizing the unique needs of our nation’s Veterans who are facing a life-limiting illness, Grace Hospice is better able to accompany and guide Veterans and their families toward a more peaceful ending. Join the Grace Hospice Journey Towards Healing 8 week Virtual Grief Support Group.
Journey Group will be held every Wednesday evening for 8 weeks beginning April 20th - June 8th, 2022 and will be hosted by the Revs John Bower, Tony Huggins and Jane Piehl, Bereavment Coordinators.
