Join the Michigan Community of Practice for Supporting Families July 26, 2022, to learn about Charting the LifeCourse, an innovative planning approach that supports planning, problem-solving and decision-making that can be used across the lifespan and in all life areas.
The Charting the LifeCourse Framework was created BY FAMILIES to help individuals and families of all abilities and all ages develop a vision for a good life, think about what they need to know and do, identify how to find or develop supports, and discover what it takes to live the lives they want to live. Charting the LifeCourse also provides professionals with a framework and tools to empower and support individuals and their families.
This virtual workshop will:
• Introduce the Charting the LifeCourse Framework as a foundation for working with people, and helping them plan for their “good life”
• Provide real life examples and strategies that can be used to develop a plan towards their “good life”
Who should attend:
Self-advocates, family members, community mental health agency staff, child welfare providers, early intervention and healthcare providers, teachers, transition coordinators, refugee and immigrant service providers, juvenile justice organizations, foster care service providers, support group organizers, medical professionals, older adult service providers, children and adult mental health providers, disability service and support organizations, social workers, independent facilitators and support coordinators, peer mentors.

• If you provide support to children, youth, adults and/or families.
• If you provide support coordination/case management to assist children, youth, adults and/or families in planning for a good quality of life.
• If you support people transitioning into different life stages across the lifespan.
