What is an important aspect of in-home health care? It's believed that helping the patient to retain their dignity is just that. What is dignity? One way of defining this is "a sense of pride in oneself; self-respect. We never want to feel helpless or that we can't do something on our own. So, our objective is to do just that. Horizon Health Care Systems, Inc. is there to provide you only the assistance you request or need. We want you to feel at home when our staff comes to visit you. Can we do a better job? Good evening,

We at Horizon Health Care Systems, Inc. would like to invite those with a vested interest in in-home care in some Southern California Regions to provide input on your community needs. We plan to do some FB Live sessions to facilitate our Strategic Communications Strategy. This strategy will allow you to contact us at any time with questions or community needs not currently provided. This page is currently monitored at least 12-14 hours a day. This is for generally information regarding the business, new ideas for service needs and general conversation. We'll do our best to respond as swiftly as possible as your questions and ideas are very important to us. We'll keep you updated when we decide to conduct the live sessions. Registered Nurses and Licensed Vocational Nurses: are responsible for the evaluation and assessment of the patient’s medical needs and will render treatment, instruction and teaching in accordance with the Plan of Care, which is duly approved by the attending Physician.

Physical Therapists: are responsible for the assessment of the patient’s physical condition and physical therapy plan in order to assist with their activities and alleviate and prevent pain, safe ambulation, transfer, balance, and gait.

Occupational Therapists: work in conjunction with other services for client to adapt function in activities of daily living.

Medical Social Workers: assess and evaluate client psychosocial needs and long-term range planning to adjust in the community.

Speech Therapists: assess and evaluate the patient’s speech defects and swallowing problems and establish an appropriate plan of care. Speech Therapists also administer speech therapy programs utilizing specialized therapeutic techniques and/or equipment.

Certified Home Health Aide: is a member of a home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed.
