This letter was mailed to all our families in April, we apologize if you did not receive it in the mail. We do not respond to Facebook posts, please call the office prior to June 30 if you have any questions. If after June 30, please see the contact address to mail the medical records request. If you have not yet signed a medical release for your child's records, please mail a request in as soon as possible. You can get a copy of the medical release from your new doctor or at our office prior to June 30. Thank you.
Dear Patients and Families,
Over the last almost ten years, we have been grateful to care for your children at Ocotillo Pediatrics. Being involved in your lives and ensuring your children’s good health has been a privilege and honor. After careful thought, we have decided to pursue other professional opportunities and will be closing Ocotillo Pediatrics as of June 30, 2018. We will not be able fill prescriptions or provide medical care after June 30th, 2018. If your child needs a physical prior to June 30th, please contact our office as soon as possible.
Your child’s record is confidential and cannot be transferred without your signed consent. You must sign a release to transfer your/your child’s records to Dr. Egbert, Dr. Foth, or another provider of your choosing.
We will be happy to continue providing care at our new locations, starting July 1st, 2018:
Dr. Egbert will be working at the Children’s Clinics Primary Care Clinic on 2600 N. Wyatt Drive, Tucson, Arizona, 85712. Phone 520-324-KIDS (5437). www.childrensclinics.org. She joins a group of established physicians who provide general pediatric care to children with complex health care needs.
Dr. Foth will be joining Callie Pediatrics at 6636 Carondelet Drive, Tucson, Arizona 85710, Phone 520-298-3383. www.calliepeds.com. Callie Pediatrics was established in 1973 and has a long history of serving the pediatric population of Tucson. Dr. Foth will join Drs. Albert S. and Albert A. Callie and Pediatric Nurse Practitioners Erin Cowan and Nicole Bencs.
The care of your children is important to us and we are free to answer your questions throughout this transition. For your convenience, we have provided a release of records so that your child’s records can be transferred to the practice of your choice. Please complete the enclosed release and mail or deliver the form to 5155 E. Farness Dr., Suite 111C, Tucson, Arizona by June 30th, 2018. After June 30, 2018 you may request records by mailing a release to Ocotillo Pediatrics, c/o Michelle Foth, M.D. at 6636 Carondelet Drive, Tucson, Arizona 85710.
