Why I don’t recommend surgery for every tear 🩺

Not every injury needs an operation and that’s especially true for shoulder tears.

Many tears improve significantly with physical therapy alone. Some partially heal, and even those that don’t can still become symptom-free through proper strengthening and rehab.

Take the rotator cuff, for example:

✅ Some partial tears heal on their own.
✅ Even full tears may not need surgery if pain and strength improve with therapy.
✅ A 6–12 week course of focused physical therapy often restores shoulder function.

Surgery is reserved for cases where pain, weakness, or limited motion persist despite consistent rehab.

For most patients, movement, not surgery, is the first step toward recovery.

Smaller scars, same spinal stability.

Here’s what you need to know about minimally invasive lumbar fusion.

Lumbar fusion is used when a herniated or degenerated disc is causing severe back pain, leg pain, or sciatica. The damaged disc is removed and the spine is stabilized with screws, rods, and a bone graft that fills the disc space.

With minimally invasive techniques, the entire procedure is done through tiny incisions, each smaller than your thumb. This approach offers major advantages:

✅ Less muscle disruption (muscles are separated, not cut)
✅ Less postoperative pain
✅ Less blood loss
✅ Smaller scars
✅ Faster recovery

For patients with persistent back pain, nerve compression, or degenerative disc disease that hasn’t improved with conservative care, minimally invasive fusion can be a highly effective option.

What I tell my patients about shoulder recovery 💪

Not every labral tear in the shoulder is the same and the treatment depends on the type of tear.

Sometimes, I perform a debridement, which I describe like trimming a hangnail. Instead of repairing the tissue, we smooth the torn edges of the labrum. The recovery is usually quick, with small incisions and minimal downtime.

Other times, the tear is more severe, pulled off the bone at its insertion on the glenoid. In that case, a labral repair is needed. Sutures and anchors are used to reattach the labrum, and recovery takes longer because the shoulder must be protected while it heals.

Each approach serves a purpose, the key is choosing the right procedure for the right tear.

When does a slipped vertebra become dangerous?

A slipped vertebra (spondylolisthesis) becomes concerning when it goes beyond simple back pain and starts compressing nerves. Warning signs include:

⚠️ Pain traveling down the legs
⚠️ Numbness around the sacral or buttock region
⚠️ Weakness
⚠️ Bowel or bladder changes, including incontinence

These symptoms indicate nerve compression that can become urgent if not treated quickly. If you notice any of these red flags, you should see a spine surgeon right away.

Will rotator cuff surgery end your throwing days?

In most cases, no.

When a rotator cuff tear is repaired arthroscopically through small incisions and the tendon is securely anchored to the bone, patients can regain full strength and mobility. With proper physical therapy and healing time (usually around 6 weeks or more), most athletes return to throwing without long-term limitations.

My take on chiropractors, as a spine surgeon 🩻

My view might be a little different from most surgeons. For the lower back, if you’re healthy and only dealing with general back pain, chiropractic care can sometimes provide short-term relief and help improve mobility.

However, when it comes to the neck, I do not recommend chiropractic neck manipulations. Here’s why:

⚠️ The cervical spine contains delicate arteries and nerves.
⚠️ Forceful manipulation can, in rare cases, cause serious complications, including stroke or nerve injury.
⚠️ The potential risks often outweigh the benefits.

So while chiropractic adjustments for the lower back can be safe in certain cases, neck manipulations are not worth the risk.

Always consult your physician before starting any spinal treatment and make sure imaging and diagnosis come first, not after.

Why are golfers more likely to need back surgery? ⛳️

Golf is an asymmetric sport. Every swing forces your spine to rotate in the same direction over and over, especially for right-handed players. That repetitive torque places heavy stress on the neck and lower back, leading to:

⚠️ Disc degeneration
⚠️ Bulges and herniations
⚠️ Chronic neck or back pain
⚠️ Sciatica or leg pain

Most golfers improve with physical therapy, activity modification, and occasionally steroid injections. But when symptoms don’t respond, surgery may become necessary because the spine isn’t built to handle decades of one-sided torque.

Why I tell patients to wait before getting back surgery 🩺

Not every case of back pain needs surgery. In fact, many patients rush into it too soon.

If you have isolated back pain without sciatica or leg symptoms, it’s often caused by degenerative disc disease, a condition that can be managed without surgery. In these cases, the best approach is:

✅ A full course of physical therapy

✅ Range of motion and strengthening exercises

✅ Anti-inflammatory medication (NSAIDs)

✅ At least 3 months of conservative treatment before considering surgery

Surgery is reserved for more serious cases like a herniated disc causing severe leg pain or nerve compression.

If it’s only back pain, wait. Commit to your therapy and conservative care first. Most patients improve significantly without ever needing an operation.

Spider veins and varicose veins aren’t just cosmetic. They’re often caused by venous reflux, a circulation issue where blood pools in the legs instead of moving upward toward the heart.

This can lead to:

Why I don’t rush into knee replacement surgery 🦵

A total knee replacement is a major procedure and it’s not truly a “new knee.” The surgery involves removing arthritic bone using a surgical saw and replacing it with metal, plastic, and cement.

It’s a great option for the right patient, but only after every conservative treatment has been tried first.

Before considering surgery, patients should complete a full course of:

✅ Physical therapy to improve strength and mobility

✅ Injections such as cortisone or hyaluronic acid

✅ Pain management and inflammation control

Surgery is reserved for severe arthritis that no longer responds to these treatments. Until then, the goal is to preserve your natural joint for as long as possible.

Do energy drinks like Red Bull or Celsius cause heart problems? 💥

The answer: They can.

Energy drinks are packed with caffeine and stimulants that can raise heart rate and blood pressure. For healthy young adults, occasional use in moderation may be fine. But for anyone with heart disease, hypertension, or hidden cardiac defects, these drinks can trigger:

⚠️ Heart palpitations

⚠️ Irregular heartbeat

⚠️ Increased blood pressure

⚠️ Chest discomfort or anxiety

Even people who feel “healthy” could have undiagnosed heart rhythm issues that energy drinks can expose. If you ever experience heart racing, dizziness, or chest tightness after drinking them, get evaluated right away.
