Raleigh Geriatric Care Management Reading Room

Raleigh Geriatric Care Management Reading Room Reading is power. Care managers are also particularly useful in helping caregivers at home find the right services and cope with their burden.

Also known as Geriatric Care, Elder Care or Aging Care Managers, a Care Manager Specialist represents a growing trend to help full-time, employed family caregivers provide care for loved ones living close by or needing long-distance care. Below is a partial list of what a care manager or geriatric care specialist might do:

• Assess the level and type of care needed and develop a care plan
• Take

steps to start the care plan and keep it functioning
• Make sure care is received in a safe and disability friendly environment
• Resolve family conflicts and other family issues relating to long term care
• Become an advocate for the care recipient and the family caregiver
• Manage care for a loved one for out-of-town families
• Conduct ongoing assessments to monitor and implement changes in care
• Oversee and direct care provided at home
• Coordinate the efforts of key support systems
• Provide personal counseling
• Help with Medicaid qualification and application
• Arrange for services of legal and financial advisors
• Manage a conservatorship/guardianship for a care recipient
• Provide assistance with placement in assisted living facilities or nursing homes
• Monitor the care of a family member in a nursing home or in assisted living
• Assist with the monitoring of medications
• Find appropriate solutions to avoid a family crisis
• Coordinate medical appointments and medical information
• Provide transportation to medical appointments
• Assist families in positive decision making
• Develop long range plans for older loved ones not now needing care

Services from care managers should be something that every family takes advantage of. Unfortunately, in reality very few families use care managers. Care managers could go a long way towards helping the family find better and more efficient ways of providing care for a loved one. The concept is simple. The family hires a professional adviser to act as a guide through the maze of long term care services and providers. The care manager has been there many times. The family is experiencing it usually for the first time. Hiring a care manager should be no different than hiring an attorney to help with legal problems or a CPA to help with tax problems. Most people don't attempt to solve legal problems on their own. And the use of professional tax advice can be an invaluable investment. The same is true of using a care manager.

Mom’s Gone Missing is not a how-to book. It’s a here’s-what-unfolded book, a daughter’s experience with a father’s decade-long Alzheimer’s journey and a mother’s lightning fast dementia decline and both of their deaths.

The narrative includes sibling tensions, an array of healthcare, financial and legal decisions, reconciliation of lifelong hurts, and the emotional tsunami that surges through it all.

This book invites you in, encourages you to find familiar thoughts and emotions, and provides compassion for your worst moments, your rawest reactions, and your urge to laugh in sometimes inopportune moments.

This true story of a thoughtful writer’s journey with her parents is written with clarity, honesty, and vulnerability. Its ultimate wish is to create a community within which to share stories and offer support. You, in whatever stage of angst or pain, are welcome here.

In this profoundly honest memoir, Daniel Shapiro shares the reality of the fifteen years he spent living and caring for his wife while also single handedly raising their three young children. During those years Shapiro searched for answers: “Can I find happiness like this? Will the rest of my life be this way?” and the moral struggle of how far to take the marital vows of “in sickness and in health.”

My Two Elaines: Learning, Coping, and Surviving as an Alzheimer’s Caregiver

One part love story, one part practical advice, My Two Elaines informs the reader on how to learn, cope and survive as an Alzheimer’s caregiver. In this book, author and former Governor of Wisconsin, Martin Schreiber watches his beloved wife, Elaine, transform from the woman he fell in love with to the Elaine who no longer recognizes Marty or their children.

My Two Elaines includes several unique features:

Excerpts from Elaine’s journal, recounting her thoughts, concerns, and frustrations as the disease progresses
Helpful takeaways for caregivers based on Martin’s observations of the disease progression
A Q&A between Martin and neuropsychologist Dr. Michelle Braun, to equip caregivers and empower them to advocate for their loved ones and their own needs

Fulfilling the role of family caregiver is difficult, even if chosen willingly as an act of love to another. While the emotional, financial, and physical toll of caregiving is well documented, the high level of self-love and self-care within the caregiver to successfully put the needs of others first without self-destructing is not.

Caregiving can be a rewarding experience for all involved, but the stress of being a caregiver can lead to burnout, exhaustion, and in some cases, financial peril IF an Aging Plan is not in place. Consequently, the stress involved in caregiving causes caregivers to put themselves and their well-being in the background and focus on their needs last. This ongoing self-sacrifice causes a phenomenon known as compassion fatigue, leading caregivers to become the "invisible patient."

In The Invisible Patient, Annalee Kruger, Concierge of Senior Care Planning, writes how to manage aging, caregiving, understand changing family dynamics, and care options (at home and care communities). Caregiving can be a positive experience IF families better understand aging, understand the disease their loved one has, learn how to improve family communication, and have an Aging Plan. The Invisible Patient provides inspiration, encouragement, and step-by-step guidance to ease the caregiving journey.

A surprise sink-or-swim lesson at the tender age of nine opens this gripping memoir of love, mental illness, and care giving. A swirling narrative carries readers from pre-WWII Illinois to the infamous Oregon State Mental Hospital of the 80s and forward along a harrowing chasm carved by dysfunctional parents, inhumane social systems, and driven by Dr. Nanette Davis’s powerful love for her mentally-ill sister and son. Raging Currents spans mental health therapies from sedation and isolation, to twelve-step programs, tough love, and modern neuroscience-driven treatments. Raging Currents offers more than an inspiring memoir: it provides practical advice and solace for modern caregivers, friends, family, and people living with mental illness.
