Welcome to Diya Care. Every day, our expert team of Diya Care Nurses, Therapists, Certified Health Aides, and Medical Social Workers provide quality care to people of every age and stage of life.

SERVICES: Private Duty : Private duty home care organizations serve the aged, ill or disabled in the comfort of their homes and communities. Personal Care: Personal Care tasks are completed by competent and trained carers and include: Bathing and Dressing, Personal Hygiening and grooming, Bathroom/Inconvenience Assistance, Light housekeeping, Physical Activities and Excursions, Grocery Shopping and Errands, Organizing Mail, Meal Preparation and Supervision, Personal Laundry and Linen Change, Ambulance Assistance and Transfers, Transportation, Dr. Appointments, Continence Management, Assistance with mobility, transfer and positioning, Assisting in and out of bed. Diya Care: Diya Care services for adults and seniors includes, short-term nursing, rehabilitative, therapeutic, and assistive care services. This is provided through hourly visits primarily paid for by the Medicare Diya Care benefit.

Diya Pediatrics: Our pediatric care takes a family centered approach to providing services for our clients under the age of 18. Diya Care Nurses and Home Health Aides are experts in caring for children requiring complex, high-tech care.

Diya Hospice: Hospice is comprehensive social, medical, and spiritual care that supports patients and their families when an illness no longer responds to curative treatments, or when a patient chooses not to pursue aggressive therapies. Hospice improves the quality of a patient’s life and provides comfort and dignity, with palliative care to help manage pain and alleviate symptoms and stress. Diya Habilitation: Our accredited Habilitation program provides support to those with intellectual and developmental disabilities. Diya Care is focused on education and therapies that empower clients to improve skills related to activities of daily living in order to participate in their community as independently as possible.
