We accept all dental insurances and are currently participating with Delta Dental, Cigna, and United Concordia. MetLife, The Health Plan, and UnitedHealthCare participation coming soon. We also accept Ohio Medicaid (excluding Paramount Advantage Plan, and Qualified Medicare Beneficiary or QMB)

Thursday, September 7, 2023 is the last day to request records. Please call and leave your name, birthdate, phone number and address for anyone that you need records.

You may also send request through fb messenger.

If you've already put in a request for records, they are being printed and will be sent to you. There's only 1 employee left getting records out, please have patience.

Please leave your name & spell it out, birthdate (if you have additional family members please leave their names & birthdates as well), phone # and the address of where you would like your records sent.

There is possibly ONLY 2 WEEKS left to obtain records. Please call the office, 740-859-4939. Once final date to obtain records is known, a post will made to inform you.
If you get the answering machine please leave your name & spell it out, leave your birth date, your current telephone #, and if you wish to pick up your records or have them mailed to you. If you want them mailed, please leave current address.
If you have multiple family members needing records please leave their names & birth dates as well.

Please be patient as there is only 1 employee working on getting records out to you.
