Heritage Park Nursing Center focuses primarily on post-hospitalization transitional care. Our greatest success is the ability to help patients recover and discharge back to home or to a lower level of care! We employ our own therapists and operate a large, fully contained therapy unit. On the Road to Recovery—With a Smile and a Goal ⚾

Meet Ms. Martin, a current resident at Heritage Park Skilled Nursing Center in Upland, CA, whose warm spirit and determination have made her a joy to work with.

After being admitted for general weakness, Ms. Martin set her sights on one goal: regaining the strength to walk independently at home so she can spend more time with her family and cheer on the Dodgers toward another World Series win.

From exercise bingo to the NuStep stationary bike, she approaches every session with enthusiasm and heart. Thanks to her hard work and the support of our physical and occupational therapy teams, Ms. Martin is now walking with a front-wheeled walker and making steady progress.

Every day is a new challenge, and Ms. Martin meets it with grit, grace, and a smile.

Keep going, Ms. Martin. We’re rooting for you, just like you’re rooting for the Dodgers! 💙

Celebrating Another Success!

After experiencing a hip fracture, Ms. Simon came to Heritage Park Nursing Center in Upland, CA, determined to get back on her feet and return home to her family—and her beloved dogs. 🐶💕

With support from our incredible physical and occupational therapy teams, she’s now walking with the help of a front-wheel walker and making amazing strides every day. Her bright spirit, playful banter, and unwavering dedication make every therapy session something to look forward to.
