Heritage Park Nursing Center focuses primarily on post-hospitalization transitional care. Our greatest success is the ability to help patients recover and discharge back to home or to a lower level of care! We employ our own therapists and operate a large, fully contained therapy unit. Progress starts with a goal. đź’™

At Heritage Park Nursing & Rehabilitation in Upland, CA, Robert arrived after breaking his arm and struggling to walk safely. His goal was simple: regain strength and return home.

With focused physical and occupational therapy, he progressed from walking just 10 feet on his first day to 200 feet with support. Step by step, his endurance and confidence improved.

Today, Robert is back home with his wife and granddaughter.

We’re proud of his hard work and grateful to our therapy team for helping him reach his goal.
