Heritage Park Nursing Center Our goal is to help patients achieve their highest level of functional independence.

Heritage Park Nursing Center focuses primarily on post-hospitalization transitional care. Our greatest success is the ability to help patients recover and discharge back to home or to a lower level of care! We employ our own therapists and operate a large, fully contained therapy unit. Together, with our nursing staff, our goal is to help patients achieve their highest level of functional independence. View a virtual tour of our facility here:

Progress starts with a goal. 💙

At Heritage Park Nursing & Rehabilitation in Upland, CA, Robert arrived after breaking his arm and struggling to walk safely. His goal was simple: regain strength and return home.

With focused physical and occupational therapy, he progressed from walking just 10 feet on his first day to 200 feet with support. Step by step, his endurance and confidence improved.

Today, Robert is back home with his wife and granddaughter.

We’re proud of his hard work and grateful to our therapy team for helping him reach his goal.

Care is not one-size-fits-all.

Our teams take time to understand each resident’s needs, preferences, and pace, providing support that feels respectful and appropriate for where they are in their journey.

This individualized approach helps create care that feels steady, thoughtful, and supportive over time.
