• Referring patient must be a current patient of Bloomfield Dental.
• Referred patients must be new patients who have not previously visited our office.
• The referred patient must complete their 1st appointment to qualify.
• The referring patient must be identified at the time of scheduling or at the referred patient’s first visit (Referral perk will not be granted anytime after the completion of the new patient appointment)
• Established families cannot receive a referral perk by adding an addition to their immediate family file (for example: adding a child or spouse to the family file does not count as a referral)
• One $10 Amazon® Gift Card per new patient referral.
• Gift cards are non-transferable and have no cash value.
• Referral rewards cannot be combined with other promotions or discounts.
