Partnership Health Center

Partnership Health Center Partnership Health Center is a charitable clinic that provides primary and urgent care for uninsured Saturday and Sunday 1:00 - 5:00.

The Partnership for Health had its beginnings in October 1996 when a small group of local business and health care leaders met to discuss organizing a local partnership that would address the changing health care needs of Lowndes County. As a result, this group developed into a non-profit corporation originally named the Lowndes County Partnership for Health, which was changed to Partnership Health Center in 2018 due to expanding nature of its program service area to surrounding counties. Since its inception the Partnership Health Center has sought to create community partnerships that address the health care needs of Lowndes County and the surrounding area. Its mission is to deliver medical and dental care to South Georgia's uninsured. The Partnership Health Center is open Monday thru Thursday from 8:00 am - 8:00 pm and Friday 8:00 am - 8:00 pm.

We’re excited to share that Partnership Health Center has received a $20,000 grant from the Georgia Charitable Care Network provided by Kaiser Permanente, to expand our Remote Patient Monitoring (RPM) Program!

RPM helps patients manage chronic conditions like diabetes and high blood pressure, through regular telehealth check-ins and closer connection to their care team.

Thanks to previous funding, 56% of RPM patients achieved significant A1C reductions, with an average 10% decrease. This new grant will enhance patient education and strengthen data collection as the program grows.

We’re proud of our Telehealth team, led by Jacqueline Pospisal, PA-C, and Megan Hoover, NP, for their continued commitment to innovative, patient-centered care.

This last week, we opened the Community Warming Shelter.

People came for different reasons—homes without heat, unsafe living situations, nowhere left to go—but they all came for the same reason: to be warm and safe. Under one roof, strangers shared space, stories, silence, and survival.

Some had a place they once called home. Some never really did. Some were escaping danger. Some were just trying to get through a cold night. Different paths, same need.

What stood out to me wasn’t just the numbers or the logistics—it was the humanity. People respecting one another’s space. Parents keeping their children close. Folks grateful for something as simple as warmth and rest.

Homelessness doesn’t always look the same, but the need for safety does.

For a moment, the labels fell away. No categories. No judgments. Just people—coming together for one common goal: to make it through the night.

💡 Streetlight Stories remind us that compassion creates space where dignity and hope can exist—even in the coldest moments.

We were honored to represent Partnership Health Center at the Lowndes County, GA Bird Supper in Atlanta last evening. Executive Director David Crawford and Director of Development Britt Eben enjoyed an evening of introductions, engaging conversations, and delicious quail prepared by Woodstack Grill.

It was a pleasure to connect with community leaders and to see PHC board members Commissioner Joyce Evans, Jess Ganas and Judge Whitesell in attendance. We’re grateful for the opportunity to be part of this longstanding tradition and look forward to returning next year.

Across Georgia, more than 1.2 million people live without health insurance. For them, getting sick can mean choosing between care and groceries, or going without care entirely. Partnership Health Center, part of the Georgia Charitable Care Network, provides healthcare with compassion, dignity, and impact. Our clinic provides care for uninsured patients at an average cost of less than $10 per patient encounter, just 8% of the cost of an average Medicaid visit.

Yesterday, Executive Director David Crawford and Director of Development Britt Eben attended the Greater Valdosta United Way Impact Partner Expo! A lovely morning spent in the beautiful Converse Dalton Ferrell House with our fellow UW Partner agencies who are making a significant impact in South Georgia.

Today was our FIRST Impact Partner Expo Day 💙

Today, we had the privilege of spending time with the incredible organizations serving our communities across South Georgia. These partners are on the front lines every day, supporting mental health, youth, families, and basic needs throughout our region.

We’re grateful for the work they do and proud to stand alongside them as we invest in a stronger, healthier community for all.
