This is hard work and great SUCCESS done by one of my clients.
You are looking at a symptom survey form....a method of tracking symptom severity and frequency. The total in the top right corner shows overall score and in this case dropped from 136 to 38!!!
Symptoms significantly reduced!!! Feeling MUCH better!!!!!
