At Fountain Park Home Health, we help make the transition from rehab to home as smooth and successful as possible. Every resident discharged after a rehab stay at a Vancrest facility receives a home health referral, and we’re honored to be one of the options they can choose.

What makes our care different

Our skilled and caring staff provides care and teaching to facilitate the independence of the individual and caregiver.
