What’s the hype … Ashwagandha?

We have heard about this supplement over the past few years - curing anxiety , helping sleep, and helping overall wellness in all aspects. So what are some facts about it ?

This is actually a shrub that grows in Asia and Africa, popular in auryevedic medicine . Studies have shown that the withanolides and alkalides may help with stress , inflammation , and sleep patterns. One thing to keep in mind is that theses herbal supplements are not FDA approved. There are cases of acute liver failure and organ damage that may occur - depending on the exact metabolites of the supplement and where it is obtained from. As with all medications - caution should be used before using , and a risk / benefit profile should be looked at with a medical provider.

Neck or Back pain from Arthritis? What else is there besides epidurals , medications, physical therapy, or surgery ?

Radio Frequency Ablation is an outpatient procedure where specific medial branch nerves that cause back or neck pain can be burned. It is not as barbaric as it seems ! - It is a 20 minute outpatient procedure that can give relief for 6-12 months or even longer. Usually we will do a test to make sure the arthritis or degeneration in the spine is actually the pain generator, and then we will set up the actual RFA. This procedure is another FDA approved option that may improve functionality and reduce pain!

Sciatica pain ?

Almost everyone has had some form of sciatica pain sometime in their life. Sciatica is not an actual diagnosis , but encompasses a variety of symptoms and possible etiologies. In general, this is pain when a nerve is irritated by a disc, bone spur, injury, or aggravation. This can cause numbness and shooting pains along a nerve pathway most commonly down the legs. Sometimes this occurs acutely but other times these symptoms can be gradual. There are many initial treatment modalities for this including NSAIDs, physical therapy/chiropractic care - but if symptoms continue an MRI is the gold standard. Following this , targeted nerve blocks or epidurals can be done to actually treat and cure the issue and avoid surgical interventions. The good news is that most people improve with these treatment modalities which are safe and quick!

Knee pain....so common, yet so complicated. There are many reasons for knee pain- aggravation of underlying osteoarthritis, degenerative joint disease, repetitive patellar femoral pain, injuries rupturing ligaments or tearing cartilage, muscular or ligamental strains, as well as traumatic injury or dislocation.

Initial workup includes physical exam, followed by possible XRay imaging and/or MRI to assess pathology and severity of injury.

Treatment options include physical therapy, oral anti-inflammatories, bracing, cortisone injections, visco supplementation gel injection, PRP (platelet rich plasma) injections, genicular nerve ablation, as well as surgical interventions.

Every situation is different and varies case by case!
